Found a great little app for FREE on my BlackBerry Apps World... and it probably exists for the iphone and others out there.
BUT, the good news, its NOT just limited to a phone, yep, you CAN use it on line too and its still FREE!!
You can track what you eat, and what you do and its geared to the USA, CANADA and other countries for your foods!!! Loving it!!!!
I downloaded it yesterday and love it!! it takes a little playing around, first go into the settings and set it up for your country, then play around in there to find all that you are looking for. And yes, you CAN add foods to it which makes it GREAT and YES you can sync it to the version on line!! whooohooo!!

You can log your starting weight, your exercise and your journals... the ONLY complaint I have is that I can't edit out some of calorie expenditures but, those are the basics that will be on any app.
